New Patient Forms
Please download and complete these forms prior to your first visit. Then choose one of these methods for submission:
1. Print and bring to your appointment
2. Email to: firstname.lastname@example.org
3. Fax to the office @ 541-808-3813.
If you have been scheduled with a Telehealth visit, please fill out this mood sheet prior to your appointment:
1. Must be approved by office
2.Please make sure we have the correct email
3.We use the zoom platform and will send a link in the email
4. Please understand that appointments can run long, allow at least 10 minutes after appointment time.
We at Compass Rose do our best to value your time and uphold your appointments. We ask that our patients do the same.
If you need to reschedule an appointment please do so 24 hrs in advance.
No shows are subject to a $50 fee as stated in our contract.
For prescription refills please call your pharmacy unless it is a controlled substance or you are out of refills. Call 541-252-8775 and leave your name and prescription needed.
Reminder: it will take 48 hours for us and the pharmacy to process your request so please plan accordingly. We do not take requests on weekends or Holidays.

For questions regarding insurance, billing, or medical questions please contact the office directly.
We accept the following insurances as In-Network providers- United(UMR), Blue-cross Blue shield(Regence), Moda, Pacific Source, CIGNA, First Choice Health, providence, and Aetna.
We can also accept other insurances as Out-of-Network providers, this may cause some out of pocket fees or copay's. It's best to contact your insurance and see in advance.
